    Please help, Ubuntu can't boot

    Hi all,
I am new to Ubuntu and need help.
My PC installed with Ubuntu 12.04, somehow it won't boot.
I've tried googling and try using suggested command below:
    sudo add-apt-repository ppa:yannubuntu/boot-repair
    sudo apt-get update
sudo apt-get install -y boot-repair && boot-repairBut Boot Repair popup only showed me "Create Boot Info Summary" instead of "Recomended Repair"
The reference is http://paste.ubuntu.com/24621837/ and http://paste.ubuntu.com/24621952/

    TIA

12.04 has reached EOL which means it is no longer supported, no updates, I recommend you install 16.04 LTS.

The I/O errors for /dev/sda shown by Boot Repair look like you may have experienced a hard drive failure. Have you checked hard drive cables? Or had you made any hardware changes?
