I normally run my screen at 3840x2160 and 30hz using DisplayPort. Occasionally after the screen times out and goes to sleep or occasionally when I power off the monitor for a while the screen is in
2560x1440 upon wake-up. I can usually use the devices->displays tool to correct this. On a few occasions, the screen came back in a much lower resolution and I had to reboot to
get the resolution back.
* I tried both ubuntu and xubuntu and saw similar results.
* Using Intel Graphics and a Asus monitor.
* DisplayPort cable
* lshw.txt attached (abbreviated a bit to fit)
Desired outcome: screen should always wake up to the last selected resolution.
