    Possible problem: card too dusty

I had the same symptoms as your first error (eth1 gone, and dmesg showed ipw2200: device failed to boot initial fw image) on my old Thinkpad X41T. In my case, taking the network card out of the laptop, blowing it, and putting it back in fixed the issue.

Before dusting the card: dmesg | grep ipw
    Code:

I had a similar issue -- an issue with the physical connection -- probably dust or a loose connection with my minipci card. Blowing it and putting it back in seems to have worked.
