After "upgrading" to 12.10 I am getting an "unsupported mode" error from my 40" Coby TV (monitor). I am getting the same error whether I use the onboard VGA connection or my Galaxy GeForce 9500 GT...
Come to think of it, I do have IA32 libs installed already so didn't notice the change... Just can't bring myself to use Chrome instead of Chromium....
Interesting, as I am running 12.04 64 bit myself with no problems.

OK, That whole experience was one big brain-fart!
In /etc/gnome/defaults.list I put in VLC.desktop instead of vlc.desktop.... One quick find/replace did the trick....

This is exactly the problem. Btnx reads only one button at a time, not one button + another button.
My suggestion is to assign your fourth button to BTN_LEFT + KEY_LEFTCTRL + KEY_LEFTALT
