I have Xinerama working on my box with a Nvidia card.
My xorg.conf follows

I initially installed the nvidia settings utility and went through the setup. In the resulting xorg.conf there is a line under the section "Serverflags" that reads
Option "Xinerama" "0"
I changed it to
Option "Xinerama" "1"
I hope this helps others, I have seen numerous people have issues with this online.
