Looking for online contacts manager

Not sure if this should be posted here, it's not related to Ubuntu. I just thought someone might have come across an application with the features I'm looking for.

I'm looking for a simple online "address book"/"contacts manager" app. It could run off the provider's site or from a server on my LAN (php/MySQL).

I wanted something similar to these two sites:

Features:
- import/export from csv
- simple
- free as in beer!! :)
