Like... a doctor's appointment or physical contact with another person?

Sometimes a physical is a time when you pee in a cup.

Could be a number of things! What were you talking about when you friend asked you? The context is everything here.

Ask her "My first physical what?" because she isn't being clear. That can mean any number of things, but my guess would be when was the first time you were physically intiment with an individual.

she is playing with you, in the context is has no meaning but it should be read to mean 'when was the first time you had sex' but only if she also does not understand english very well.

"What is your first physical" has mixed tenses, 'is' is a current tense but the context suggests a past tense. it should have been "What was your first physical" also the word physical means "Composing of matter, a real entity in the world which interacts and exists." so you can see how it doesn't make sense unless you imply missing words:

Physical Exam - As stated above, a medical examination by a doctor of nurse
Physical Act - Strictly the first movement you've done, can mean first sexual act
Physical Sport - The first sport you every played
Physical object - Brings it back to present tense and asks for a preference.

It's hard to tell really what the real meaning is.
