What gtk and shell theme are you using?
None. I don't use any DE.
I have a custom setup using fvwm which controls window details, title bar buttons, and placement of those buttons. Anything that can be seen, can be customized. Colors, thickness, behaviors. I suppose it supports themes? Yep: http://fvwm-themes.sourceforge.net/ I've not used those in any significant way.

DE: Xfce
WM: Xfwm4
WM Theme: OlliFri-XFCE-Dark-38px
Theme: Eye-friendly-Dark-M-RBC [GTK2], Arc-Dark [GTK3]
Icons: Delft-Audacious [GTK2], Sardi-Arc [GTK3]
Gave up on theming Gnome - until a good tutorial anyway!
