Alfa AWUS036ACH USB Wireless Adapter not working

I've been trying to get my new Alfa USB WiFi adapter to work for the last 2 days with no success. I tried with both the default rtl8812au-dkms package from the 18.04 repositories, and the updated driver from https://github.com/kimocoder/rtl8812au. In both cases, I am able to see my wireless network in network-manager, and connect to it - but my gateway is unreachable. Here's the output from @jeremy31's wireless-info script:

Ok, this is weird... everything seems to be working now. I ran:

... and my adapter connected. Hopefully this configuration stays stable, but it would be nice to know how it started working.

After many months of working fine, the adapter has suddenly stopped working again. lsusb shows it connected. I've tried rebuilding and reinstalling the driver, but no luck.

I'd appreciate any guidance on how to go about diagnosing and fixing this problem...
