This is annoying and confusing. How do I disable it? ....
When I am in a program and I go to Load or Open a file, I am always started at "Recently Open" instead of Desktop, Home, or Computer. I'd prefer to either get rid of "Recently Open" altogether or at least default it to go to one of the other places. I'm seeing double files here!

Typically each application will have its own Recently Opened settings. For others such as gedit, you can edit the gconf-editor--search for "recent".

Set the "Recently Opened" files to 0, then restart the application.

What version of Ubuntu and what applications are you having problems with?
