I am following the instructions, but am getting the following error of loads of items not found. What should I do please? fix-missing brings more 404's....

Welcome to the Ubuntu Forums :-)

Your problems could be due to the old age of Ubuntu Server 8.04 LTS. Even though it had long time support, it was limited to 5 years from April 2008, so end of life was April 2013, almost one year ago, and the repositories for updating and upgrading may not be available now.

I don't know enough to help you find what you need, but there are many people (volunteers) at the Ubuntu Forums, who know a lot about Ubuntu Server and about updating and upgrading problems ... with a little bit of luck someone who can help you will read your thread. If no response within 24 hours, you can bump the thread (write 'bump' in a new post).

Still I would suggest a backup and a fresh install of 12.04.
