I upgraded from 12.10 to 13.04 and ran into this problem. I then did a clean install of 13.04 and still have the problem.
Dual screens worked perfectly under 12.10, but they are flaky under 13.04. I can mirror most of the times, but in extended (non-mirrored), the launcher bar and status/menu bars disappear.
I've figured out a work around where I alt-ctrl-del to logout and then come back in as mirrored, then switch to extended. After 1-2 times, it seems to stick and I'm good until reboot again.
