I want to know whether I can include my country name while choosing the professional email id?

huh?
you mean that the country could be known from e-mail address? no it's not necessary.
if you want to you could create domain with your country name in it. e.g. bbc.co.uk

Which is more professional?
firstname.middleinitial.lastname
or
firtname.middleinitiallastname

firstname.lastname unless you use middle name.
or you can also have only first name. edit: it's easier to remember for clients.
your email if propperly set will have full name anyway. something like:
FirstName MiddleName LastName - firstname@example.com
or
FirstName MiddleName LastName [firstname@example.com]

firstname.lastname or firstname username is not available.
And only available choices are:
firstname.middleinitial.lastname@example.com
or
firstname.middleinitiallastname@example.com

tough choice then as they both suck. what kind of service provider is this? :-O
go for second one it might be easier to spell it out if you need to tell it to someone over the phone.

I want to use gmail only as it is somewhat integrated with many services.

and gmail doesn't have more options?
sorry, but when i first signed up with it i did it via invite. it was a long time ago...things must have changed since then.
anyway i remember manually creating email addresses not so long ago, but we did pay for google apps/domain. so it could be the options are different there.
