Unusual icon on desktop

When I turned my computer on last week, there was a new icon on my desktop. It appeared to be a screen shot of a website that I use frequently. Both the icon and folder that opened when I clicked it had a heading that was part of my password for that site. Have I been hacked?

It may help to understand the problem if you could post screenshots.

if you have been hacked there are two things that could of happened one some stuff is in a diffident place hardly noticeable maybe a space away...
or it could be that you created a short cut to a website by accident
Oh and don't log on or do it with a spam account that can burn for all you care.

Here's what I'm thinking:
1. you were logging into the site
2. you accidentally hit the hot key combo which takes a screenshot while you typed your password
3. you finished typing your password but you were actually naming the screenshot
4. you hit enter when you were done typing your password which saved the new screenshot onto your desktop.

Could that be a possibility?
