maximus is totally inconsistent

I'm using gnome shell in 12.04 and maximus only maximizes windows some of the time. From what documentation I can find maximus should maximize *per application* so it should not vary from session to session. any ideas?

This extension just removes window decoration. Maximus is doing that just fine. What it's not doing is maximizing all windows.

Anyone?
