I'm using the Gnome 3 in Classic Mode (no effects), and there's at least 3 problems in my Top Menu:
1. The Update Monitor icon isn't showing up properly (how to fix the icon?)
2. There's a blank space between the Sound and Network icons (how to remove the space?)
3. The Network icon is bigger than the standard (how to resize it?)
4. I don't see any simple way to remove the icons

The universal access icon sits to left of speaker indicator . Do you have dark indicator on a dark background perhaps ?
Try another icon set and see if the problems are related to the icon set you are using .

Thanks but it's not the Universal access icon. It's a blank space for sure.

I just tried, and all the problems remained.

If at least there was a simple way to remove the launchers at the top right menu.
