we'd like the subdomain "ubuntu-na.ubuntuforums.org",
The LoCo team name should be "Ubuntu Namibia LoCo Team"
Forum administrator will be "gjensen" (myself),
registered team contact will be myself as well (<snip>).

I have created the forum for you and made you a LoCo Moderator for that forum. The subdomain may not be working just yet, but the people who can sort it are aware, so I'll post back here once that's working too.
