We don't :)

But if you really, really need it, use GIMP and relevant plug-ins such as Slice Tool from gimp-sharp.sf.net or the Slicer bundled with GIMP.

You might want to use an image map in html:

An application like dreamweaver would let you draw your image map directly on the image, and you can export to html.
