sudo apt-get install pdfedit

This looks ok

You might want to look heree - http://www.cyberciti.biz/tips/open-source-linux-pdf-writer.html before you install to check it's what you want

Open office can't do that?

Edit: Have a lok into this guide: http://danilop.wordpress.com/2008/07/02/how-to-create-pdf-forms-with-openoffice/

A bit old but might just work and be the thing you are looking for.
