Lost nautilus file manager icons

I installed the elementary theme and played with it. Unfortunately, I accidentally remove the icons in the file manager. Refer to attached image.

How will I display the icons, menus and address bar again?

Can somebody help me with this problem? I wanted to reinstall Ubuntu OS to solve this problem but if you have solutions please share.

It's been a week since you posted your question. Did you end up finding a solution, reinstall reinstall Ubuntu?

If not, had you previously installed Nautilus Elementary (from the ppa:am-monkeyd/nautilus-elementary-ppa repository) in addition to the elementary theme?
