Upgrde from 8.04.4 LTS to 10 LTS

I'm looking for advise on the correct upgrade process path , to upgrade an Ubuntu 8.0.4.4 LTS server running on xen to 10 LTS .. Is there a document / Howto on doing this ?

The server is a live server so I would like to avoid any major crashes and having to resort a disk image.
