Try e-mailing him. It's no longer on his website so he may no longer wish to publicly support it (or make it available) but he may still have a copy around.

I've done a quick search, but haven't bumped into it yet. We're gonna have to start archiving some of these scattered resources.

I've asked in linux-nasm-users Yahoo group... someone there is likely to have this.
