I don't understand why are there so much cursor themes on gnome-look.org , but there's no tutorial about creating a new theme anywhere. I'd like to know how to make different sizes on the same icon, and which name needs each one (since I see in some themes names as 03b6e0fcb3499374a867c041f52298f0).

You're right about a complete how-to, but I imagine with the above you could probably sort it out by copying a previous cursor pack and replacing images by creating your own PNG's in GIMP or the like. Hope that helps!
