The deadhand from Legend of Zelda: Ocarina of Time. There are 6-10 hands sticking out of the ground, and you have to let yourself get grabbed by one. While you're struggling to get away, this dude pops out of the ground, comes over to you, and tries to eat you.

American McGee's Alice has some messed up bosses - the Jabberwocky, the Mad Hatter, and of course - the Queen of Hearts. After you "defeat" her, her true self appears, and you finally understand what all those tentacles swirling around all levels are... Too bad I can't find any pics.

Also, the Wizard of Yendor from Nethack may deserve a place on the list. He is not a mortal threat if you are well prepared. But the first time you meet him you are in such an adrenaline rush it is scary. And he is mighty annoying (he is immortal, reappearing shortly after his death to bug you further).

Dude, I'd forgotten about the Dead Hand. I loved that boss because of how freaky he was.

Xel'atoth from Eternal Darkness. Not so much herself as a character but for the fact when fighting her your character is quickly driven insane and the insanity itself can be a little bit... disturbing.
