Checking what outbound connections an app is making

How can I check what outgoing connection(s) an application is making, and possibly restrict that connection until I approve it manually?

My intention isn't to limit connections, but a way of determining what an app is doing (if anything) remotely.. i.e., who it's connecting to, what it's sending, is it listening on any port, etc..
