How to convert video with these specification

I"m not to familiar with ffmpeg or converting video using cli. Here are the specification i need the ouput video to be in.

Stream #0.0: Video: mjpeg, yuvj422p, 640x480, 30.01 fps(r)
Stream #0.1: Audio: adpcm_ima_wav, 8000 Hz, mono, 32 kb/s
