Uploading of images problem.

Quite often when attaching an image I will get an error message failed to upload.
These are jpg's well within the dimension and size constaints and will often show as uploaded
after just refreshing the attachment window 2 or 3 times.
Also the order of attachments when shown in the post seems to be by size and not upload order.

+1 re. the size issue. I see the same quite often with .png files smaller in dimensions and kB than needed to trigger the failure to upload
