    "failed to create a file system"

    installed ubuntu 12.04.3LTS on desktop via usb. chose to install on desk top and wipe out windows xp. went great for few days now will not start up without usb reboot stick. once and if it starts up. i tried 2 re-install from usb.
    get to the "where are you?" point of instullation and screen pops up

    "failed to create a file system
    The ext4 file system creation in partition #1 of SCS13(0,0,0) (sda) failed"

    can"t get past this point or at least cant get it off the screen. what did i do what do i need to do systen emachew3644 was runnin windowws xp

    How did you create the usb live stick?
