Now that there is this update, must I (a 12.04.1 user) download this iso and do a fresh install or just do a system upgrade?

No need to reinstall. 12.04.2 is simply a convenience for new users; it includes all the updates since Lucid was released in April 2012. If you've been keeping up with your Update Manager then you already have these updates.
