    Ran it again and it did not fix the entries.
    So I tried

    Sources can also be found in:

    Code:
    /etc/apt/sources.list.d
    This may be where the duplicates are and they may be spread over more than one file.

    I got nothing in that directory even with view hidden files turned on

    Ok, I'm not an expert with sources.list, could the following be your problem

    These lines:
    Code:
    1 deb http://us.archive.ubuntu.com/ubuntu/ precise main restricted
    1 deb http://us.archive.ubuntu.com/ubuntu/ precise multiverse
    In the line below they are repeated because you are asking for restricted, main and multiverse which you already have in the lines above:
    Code:
    1 deb http://us.archive.ubuntu.com/ubuntu/ precise restricted main multiverse universe
    And this is a repeat of part of the line above
    Code:
    1 deb http://us.archive.ubuntu.com/ubuntu/ precise universe
    What happens if you comment out the following line?:
    Code:
    deb http://us.archive.ubuntu.com/ubuntu/ precise restricted main multiverse universe
    From what I gather the lines in the sources.list are defined like this:
    Code:
    deb uri distribution [component1] [component2] [...]
    In your sources.list you are repeating the same components on several lines; main, restricted, multiverse and universe
