Headset only works once. Requires a reboot or lightdm restart in order to function.

Never had this problem before. Always worked excellent for me.

Upon a fresh install of 12.04, my headset will only work the first time it has been plugged in. If I unplug it and use it later, it will not work unless I restart Lightdm (logging out won't fix it) or restart the PC.

I can still see and select the device in the GNOME audio controls. It just doesn't do anything, and continues to use my speakers instead. Really annoying thing!

Edit: Just switched around some of the ports and got a black screen. Loads of cryptic error messages in a TTY regarding Xorg being tainted. Maybe this is an Fglrx issue? (Had to hard-reboot)
