Sounds like the drive is dying ...
You write stuff to it, sectors go bad and eventually, something needed by the OS gets corrupted.
Then, you reformat, reinstall, and it happens again.
When sectors go bad, the hard drive controller marks those sectors as unusable, so when you reformat, those bad sectors are not used again.
Since this keeps happening again and again, my guess is that additional sectors keep failing.
If you can find out the make and model drive, you can visit the manufacturer's website to see if they offere a downloadable utility you can use to check the health of the drive.
