how do I build a "chooser" style website?

I really like the Linux Distro Chooser. I have emailed the developer several times and heard nothing.

I run a small soccer club in Chapel Hill. We have a lot of parents ask us questions about which of the local clubs is appropriate for their child to play for. I want to setup something like the above chooser site. But I have no idea how.

Does anyone have any tips? or is there a template, theme or existing service that you could recommend?

a Google Docs Form can do that, if you dont know coding enoght...

I suspect it's a custom application written in PHP.

I've written an application like this some years back, but it was also a custom job.
