I am not able to troubleshoot that one. Try replacing it with:
sudo grub-mkrescue -o ~/live-cd.iso ${CD}
And see if it makes any difference.

The thing that puzzles me is why didn't they create unity in top of gnome-shell using its extension system? It would have much less effort and easier to maintain.
Since moving to linux I have never reinstalled my system. Even when I break my system, which happens rarely, I restore it back from a tarball backup.
Why not a GUI to manage partitions instead of manually editing fstab?
One more thing. To be sure this is not a bug in casper, you can try using ubuntu's official ISO on your usb stick and see if it works. If it does not you can file a bug report to ubuntu devs.

If you run the above command and it does not do anything, it means you have nothing to worry about.
