    ok... ran the SMART self-test and /dev/sdb failed with a read error ... so how do i identify which physical drive is /dev/sdb ?

    just mounted the RAID and it said there were filesystem errors. running fsck fixed the errors, but dmesg also contained errors like posted above. does ata4 (as in the error) correlate to SATA port...

    That is absolutely true. My case was a weird one though, because i was trying to upgrade disks.
    I'm going on memory here, but i think i ended up with the 2 original disks and the new disk all...

    *** EDIT *** : using --zero-superblock should not be used in this case. --assume-clean is better for this application. It worked for me, but apparently was a bad idea.

    I ended up formatting the USB and repartitioning, making ubuntu the first partition, and removing the YUMI installations.

    Once this was done, it was easy to setup what i wanted to do with grub.
