I was able to install the downloaded firmware-b43-installer package from a USB stick while running from the current live CD.

None at all. I have installed 11.10 by upgrading an 11.04 cmi installation "do-release-upgrade" and then installing either the Ubuntu or Xubuntu Desktop via the "tasksel" command.

Have you tried to use the entire disk rather than erasing 11.10 and reinstalling into the old partition? Since yaboot seems to have trouble finding your entire harddrive repartitioning it might help....

The 11.10 mini iso I downloaded did not recognize the ibooks hard drive during install. I don't know if that has been fixed by now.
