I have been able to get my cinema display working as second monitor with my macbook pro and fglrx. During startup the monitor mirrors what is on the laptop display, but once X starts it goes black. The light that normally indicates that the monitor is being used remains on. I ran the following command:
aticonfig --initial=dual-head --screen-layout=above
which gave me the following xorg.conf:
