I currently know of DreamHost (http://www.dreamhost.com/r.cgi?arthemys).
But they are using Debian, so that doesn't exactly answer your question, but it is close. Any they do a great job at it too, that's where my site is hosted and soon will be hosting my company's website.

I, along with several other Ubuntu users use Linode. They support Ubuntu out of the box.

For a VPS provider such as Linode, there's unixshell# (http://unixshell.com) that provide Ubuntu (both versions) as an option.

Ive started using EHCP which is a great package. Installed no problem. Im hosting three websites now, but I can add as many as I want. Im using Ubuntu 8.04 and it runs like a champ.
