why not just use gthumb?

Because with gthumb you can only extract the 128px icons and one at a time, with the utility posted in the link you can extract many sizes and faster.
Most of the icons on the iconfactory have different sizes and they don't just scale the icon from 128px, they actually design different icons for sizes smaller than 48px.

its only for windows but gnome surley needs one its alled : iphile.
it presents a list of sysmte objects and their icons. you drag n drop images onto the objects and save the settings as a profile.
just like gnome..except no system-object-borwser...have to do this individually to any .desktop object you see in nautlius.....one at a time
also regarding png not having multiple layers...have a look at macromedia fireworks the image format is PNG...and they use layers, which can be utilised if imported to freehand or illustrator or photoshop. (i doubt its a proper png format though)
