Howto handle upgrades?

I'm new to mythbuntu, I've been using mythtv with fedora and gentoo. I have general question about upgrades are those applied on the fly (both application and operating system) or do I have to do a fresh install every time?

Do you mean upgrades from one release to another (ie, 9.04 to 9.10) or upgrades to new packages (ie. mythtv-0.21-fixes21119 to mythtv-0.21-fixes21134)
