mx510 mouse with enemy territory

I have my mouse working in X but when I try to configure my mouse in ET I can only use some of the buttons. I have smartscroll disabled so that I have access to those extra buttons.

When I try to use the window switch button or the two smart scroll wheel buttons it uses KP_EQUALS. My thumb buttons work fine with makes it seem even stranger.

I also noticed that my scrollwheel button is MOUSE3 not SCROLLWHEEL something, I don't know if this is normal or not.

Does anyone know why I am not able to use all my mouse buttons?
